*Membership Madness Prize Package Sweepstakes Official Rules

1. No purchase necessary.  A purchase transaction will not increase your chances of winning.  You have not yet won. Void where prohibited. Open to legal residents of the 50 United States and District of Columbia who are 18 years of age and older at time of entry, except for employees, board members, and immediate family members or persons living in the same household of Park Community Credit Union. All federal, state and local laws and regulations apply. Starts March 11, 2026 and ends on April 24, 2026.

2. Eligibility for Promotion:

Automatic Entries – Any person who submits a ‘Planning ahead with Park’ survey response between March 11, 2026 and on April 24, 2026.

Manual Entries – Members in good standing and non-members may enter by printing your full name, address, telephone number with area code and email address on a postage-paid 3” by 5” card and mail it to: Park Community Credit Union Membership Madness Sweepstakes, Attn: Marketing, 2515 Blankenbaker Pkwy. Louisville, KY 40299. Only mail entries received during promotional period will be accepted. Hand-delivered entries will not be accepted. Mechanically and electronically reproduced write-in entries or entries submitted via any third party not directly affiliated with the Credit Union will not be accepted. Regardless of entry method used, entries are limited to a maximum of one entry per Tax ID for the promotional period. Additional personal information may be required for IRS reporting. Entries become the property of the Credit Union and will not be returned. The Credit Union is not responsible for lost, late, illegible, misdirected, mutilated, incomplete or postage-due mail entries. Incomplete or illegible entries will be disqualified. Members or non-members submitting multiple entries will be disqualified. All automatic entries and manual entries will be referred to herein as “Entries” or each as an “Entry”. Persons submitting Entries are referred to herein as “Entrants”.

Legal U.S. Residents only. Must be 18 years or older to enter. Business members are not eligible. Credit Union employees and volunteers, and their immediate family members, are not eligible. A “Member in good standing” means no delinquencies on any Credit Union loans, no collections, negative share balances, excessive non-sufficient funds, charge-offs or loans in forbearance at time of entry and drawing. In the event of a dispute as to the identity of a potential winner or Entry based on automatic entry, the member who is the primary account holder will be declared the winner, as determined by the Credit Union’s records.

Prize – Winner(s) will receive a prize package. Package may comprise of tickets to local events and experiences, Park branded gear, or other items to be determined by Park Community Credit Union – FMV: approximately $250.

No substitutions or transfers of prize(s) are permitted by the winner(s), except at the Administrator’s sole discretion. All taxes and all other costs associated with acceptance or use of the above prize(s) are the sole responsibility of the applicable winner.

3. Notifications and Release: Potential winners will be notified by phone, U.S. mail, or email by the Credit Union within seven (7) days of the drawing. If any prize is unclaimed within seven (7) business days of notification, an alternate winner will be randomly drawn. All potential winners must comply with all terms and conditions of these Official Rules, and being declared an official winner is contingent upon fulfilling all requirements set forth herein. Potential winners may be required to sign and return an affidavit of eligibility and compliance (including providing their Social Security Number for tax purposes) and a publicity/liability release, as allowed by law, within seven (7) business days of notification attempt or they will forfeit the prize. In the event of forfeiture, an unreachable potential winner, a disqualification or if the potential winner declines acceptance of the prize, the Credit Union may award the prize to a randomly selected alternate potential winner. All potential winners are subject to verification by the Credit Union whose decisions as to selection and verification of potential winners are final and binding in all matters related to the Sweepstakes.

4. Consumer Disclosure: Odds of winning are based on the number of entries. For example, if there are 1,000 entries your odds of winning would be 1 out of 1,000.

5. Tax Responsibility: Winners will receive an IRS 1099 Tax Form in the mail for the value of the prize. Any member or non-member who wins prizes through any Credit Union promotions with cumulative value equal to or greater than $600 in a calendar year will receive an IRS 1099 Tax Form. Prize winners are solely responsible for all applicable federal, state, local or other taxes, assessments, fees or costs associated with or imposed upon the winner of any prize in this promotion. No prize substitutions or transfers permitted except at the sole discretion of the Credit Union.

6. Release of Claims: Entrants, winners, members of any entrant’s or winner’s household, and their respective relatives and persons who use any prize awarded by the Credit Union, and such persons’ heirs, assigns, executors and administrators release, indemnify and hold harmless the Credit Union and its directors, officers, employees and agents from any and all claims, actions, demands, choses in action and damages which they have now or any time in the future arising from the use of any prize, including personal injury or death. IN NO EVENT WILL THE CREDIT UNION AND ITS AGENTS BERESPONSIBLE OR LIABLE FOR ANY DAMAGES OR LOSSES OF ANY KIND, INCLUDING DIRECT,INDIRECT, INCIDENTAL, CONSEQUENTIAL OR PUNITIVE DAMAGES ARISING OUT OF YOURPARTICIPATION IN THIS SWEEPSTAKES OR ACCEPTANCE OR USE/MISUSE OF ANY PRIZE.

7. General Conditions: By entering the Sweepstakes drawing and/or accepting a prize as a selected winner, each entrant/winner agrees that they have read these Official Rules and agrees that they will be strictly bound by them. The Credit Union’s decisions shall be final in all matters pertaining to this Sweepstakes and any/all prizes awarded. Further, entrants/winners grant the Credit Union the right to use their individual name, addresses, photographs, and likeness for advertising, publicity and promotional purposes and for any other purpose as determined by the Credit Union, in any and all media without limitation, without further compensation or permission, except where prohibited by law. The Credit Union reserves the right to cancel, suspend, and/or modify the Sweepstakes at any time without notice. The Credit Union reserves the right, in its sole discretion, to disqualify any individual found violating these Official Rules or behaves in an unsportsmanlike or disruptive manner. Entrants/Winners agree that the Credit Union and its officers, directors, employees and agents have not made nor are in any manner responsible or liable for any warranty, representation, or guarantee, statutory, express, or implied, in fact or in law, relative to this Sweepstakes or to any prize. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of an Entrant and Credit Union in connection with the Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of Kentucky, without giving effect to any choice of law or conflict of law rules (whether of the State of Kentucky or any other jurisdiction).

8. Sponsor: Park Community Credit Union is the sponsor of the drawing. The promotion is in no way sponsored, endorsed, administered by, or associated with any other sponsor.
